The form is filled out by selecting the level of sponsorship, indicating the hospital of their choice, and submitting the form to the hospital's financial, purchasing or endowment department and a copy to Ceiling Scenes. Benefactors can designate their gift in memory of a loved one or on behalf of an individual or organization. They can also choose to remain anonymous.
